The best mattress for back sleepers is medium to firm. That means firm enough underneath that the hips stay level with the shoulders, with a thin comfort layer filling the curve of the lower back. A back sleeper is anyone who spends most of the night face up, and in that position the pelvis presses deepest into the bed. On a soft bed the hips sink and the lower back arches. On a very hard one the small of the back hangs over a gap.
The research on beds and back pain points the same way. In a 2003 trial in The Lancet, people with chronic low-back pain did better on a medium-firm mattress than on a firm one. Canada's drug and health technology agency, CADTH, reviewed the evidence in 2022 and reached a similar, carefully hedged view.
Merit Home Furniture lists 14 mattresses for its Duncan store on Vancouver Island, 8 from Restwell and 6 from Restonic. Their makers call 4 of them firm and 3 medium, which gives a back sleeper a short list of beds to lie on. Below are the studies, the beds, a store test, and the mistakes that turn a good mattress into a sore back.
Is a soft or firm bed better for your back?
For back pain, a medium-firm bed has the best evidence behind it, and neither extreme does well. Four pieces of research carry most of the weight. The table gives what each one measured, in the authors' own terms.
| Research | What it found |
|---|---|
| Kovacs and colleagues, The Lancet, 2003 | 313 adults with chronic low-back pain were given medium-firm or firm mattresses for 90 days. The medium-firm group had better odds of less pain in bed (odds ratio 2.36) and less disability (2.10). |
| Radwan and colleagues, Sleep Health, 2015 | A review of 24 controlled trials. A medium-firm mattress that the sleeper can adjust (custom inflated) came out best for comfort, sleep quality and spinal alignment. |
| Caggiari and colleagues, Journal of Orthopaedics and Traumatology, 2021 | A review of 39 studies. It found a medium-firm mattress "promotes comfort, sleep quality and rachis alignment" (rachis is the spine). |
| CADTH, Therapeutic Mattresses for Chronic Pain, 2022 | Drew on one review covering 9 relevant studies. Medium-firm mattresses "may lead to less back pain and better sleep quality" than firmer ones, with limited certainty. |
The Lancet figures come from a European firmness scale that runs from 1.0, the firmest, to 10.0, the softest. The firm bed in that trial scored 2.3 and the medium-firm bed 5.6, so medium-firm sat close to the middle of the scale. None of the makers on our list prints a number on that scale. Their "firm" and "medium" are their own labels, and one maker's medium can lie quite differently from another's.
Two cautions keep the research in proportion. The people in these trials had back pain and were not sorted by how they sleep. The studies back medium-firm for sore backs, then, more than for back sleepers as such. CADTH also notes that several limitations in the data limit the certainty. Still, the evidence points to medium-firm, and for a back sleeper without pain the same answer follows from alignment. CADTH puts the stakes plainly: one in 5 people living in Canada has chronic pain.
How a back sleeper's spine sits on a mattress
A back sleeper's spine has a gentle inward curve at the waist, and a good mattress fills it without pressing it flat. The comfort layer on top does the filling. The coils or foam core underneath stop the pelvis from sinking below the line of the shoulders. That keeps the curve from deepening into an arch over a long night.
HealthLink BC, the province's health information service, gives two small aids for sleeping on the back with back pain. Its first is to "place a pillow under your knees". Its second is a small rolled-up towel under the curve of the back. Both ease the lower back, and both are worth knowing about whatever bed is underneath. Neither one fixes a mattress that lets the hips drop.
The pillow counts for nearly as much as the mattress. HealthLink BC's neck page asks for a pillow that "doesn't force your head forward" when lying on the back. A tall, stiff pillow tips the chin toward the chest, and a firmer mattress makes that tilt more noticeable because the shoulders sink less.
A back sleeper who also turns onto the side for part of the night is better off at the softer end of medium, so the shoulder has somewhere to go. Pure side sleeping is the opposite case: the shoulder and hip need a softer top that lets them sink in. A back sleeper who stays put all night can go firmer.
The check I use is a flat hand. Someone lying on their back slides a flat hand under the small of the back. If it goes in with slight resistance, the bed is filling the curve. If it slides in easily, the bed is too firm for that body; if it won't go in at all, the hips have sunk and the bed is too soft.
Medium and firm mattresses for back sleepers at Merit Home Furniture
These are the Restwell and Restonic beds listed for our Duncan store that their makers call firm or medium. Prices were read from the live listings on 16 October 2026, before applicable taxes. In queen they run from $899.00 for the Canary to $2,599.00 for the Vernazza. Several come in only a few sizes, so a quick call confirms the size wanted. Mattresses take 2 to 3 weeks to be ready for pickup or delivery.
A tight top has no separate padded layer sewn on. A Euro top adds a thin extra layer sewn flush with the sides. For a back sleeper, both work when the overall feel is medium or firm.
| Firm mattress | Build | Listed price |
|---|---|---|
| Restwell Canary | Firm Top, tight top, 1,200-count 5-zone pocket coil | $899.00 queen |
| Restonic Minto 12.5 | Sleeps firm overall, Euro top over a foam-encased zoned pocket coil | $1,199.00 queen |
| Restwell Nightingale | Firm Top, tight top, micro coils and zoned latex over a 5-zone pocket coil | $1,499.00 queen |
| Medium mattress | Build | Listed price |
|---|---|---|
| Restwell Starling | Medium Top, Euro top, micro coils and zoned latex over a 5-zone pocket coil | $1,449.00 queen |
| Restonic Notte | Sleeps medium overall, Euro top, copper memory foam over a zoned pocket coil | $1,688.00 queen |
| Restonic Vernazza | Sleeps medium overall, Euro top, Talalay latex and Joma wool over a zoned pocket coil | $2,599.00 queen |
The Minto is a useful case for reading labels. Restonic says it sleeps firm overall even with a Euro top, since a firm mattress still uses firmer or softer layers inside it. For a couple where one sleeper wants more give, the Starling and the Nightingale share Restwell's Elite tier, one a Medium Top and one a Firm Top. The Nightingale also has a second listing, the Nightingale Elite, with the same model number at $1,449.00 queen, so a call settles which price applies.
Zoned pocket coils
A coil bed gives a quicker push back and an easier roll from back to side. It also tends to sleep cooler than solid foam and to hold a firmer edge. On the Canary, Restwell lists firmer coils under the lower back, the zone that sits under the hips. Restonic zones the pocket coils in the Minto, Notte and Vernazza.
All-foam TEMPUR beds
An all-foam bed moulds closely around the body and passes less movement to a partner. Rolling over takes a little more effort. The beds above that list their build all have pocket coils. A back sleeper set on the all-foam feel can ask our Campbell River store for its best price on the Tempur-Pedic Align 10 Medium, an all-foam tight top.
How to test a mattress for back sleeping: edge support, the roll and time
Testing a mattress for back sleeping in a store takes longer than a quick bounce. The review sites score how each bed performs during testing, with lab checks for sinkage and an edge support test. A store visit can do the same work on the body that will actually sleep on the bed. This is the order that makes sense to me.
- Time on the back. A few seconds tells you about the fabric. Several minutes lying still lets the comfort layer settle under the hips.
- The hand test. The flat hand under the small of the back, described above. It gives the same reading on any brand.
- The roll. Rolling from back to side and back again shows whether the bed lets go easily or holds the hips in a dip.
- The edge support test. Sitting on the edge, then lying along it. A foam-encased edge, which Restwell lists on the Canary and Nightingale, keeps more of the surface usable and makes getting up easier.
- The home pillow. Bringing the pillow from home shows how the head and neck sit on that particular bed.
- Both sleepers. When two people share the bed, each one does the hand test. A medium that suits one back can leave the other hanging.
Merit's Duncan floor holds display models, and most mattresses are ordered. A call ahead tells you which of the beds above are set up to lie on that week.
What are the best mattresses for back sleepers according to reviews and chiropractors?
The online review sites mostly pick boxed beds sold direct, from brands Merit doesn't carry. Their scores still teach something useful. Sleepopolis says it has tested more than 500 mattresses, and it rates two of its back-sleeper picks, one all-foam and one hybrid, 6.5 out of 10 for firmness.
On the Sleepopolis scale, 1 is softest and 10 is "as firm as the floor". That runs the opposite way to the European scale in the Lancet trial, where 1.0 is the firmest. Both scales put a good back-sleeping bed near the middle, which is the same answer from two directions. A reviewer's 6.5 is still one tester's body on one bed, and a lighter or heavier person may rate the same mattress differently.
On chiropractors, the trials behind the usual advice compared levels of firmness, not brands. So the mattress a chiropractor recommends is really a feel, medium-firm, and several makers build one. A chiropractor, physiotherapist or doctor who has examined a particular back can say far more than any list. For buyers who want a Canadian maker, Restonic's western plant is in Surrey, BC.
Where a back sleeper's mattress choice goes wrong
What mattress should a back sleeper stay away from?
A thick, soft pillow top. However good the coils underneath, a deep soft layer lets the hips through. Makers label these plainly, as Restwell does with the Sparrow's plush pillow top, and they suit side sleepers far better.
Extra firm on the theory that harder is better for the back. In the Lancet trial the firmer bed did worse. Not one of the 14 mattresses listed for Merit's Duncan store is called extra firm by its maker, and even the firm ones deserve the hand test first.
A tired base. A firm mattress on an old box spring or a sagging frame sleeps soft in the middle, and the dip lines up with a back sleeper's pelvis. The signs that a mattress or base has worn out are set out with how long a mattress really lasts.
None of this is medical advice. HealthLink BC says most back pain gets better within 4 to 6 weeks with basic self-care. It lists pain that wakes you from sleep, new numbness or weakness in the legs, and new loss of bowel or bladder control as reasons to call a doctor now. Back pain with chest pain or other heart attack symptoms, or signs of spine damage after a fall or car crash, means calling 9-1-1. If pain is the reason for buying a bed, talk to your doctor or a physiotherapist first.

Trying a back sleeper's mattress in Duncan
The Merit store in Duncan, at 107 Ingram St, is open Monday to Saturday, 9 to 5. Trying three of the beds above, with the hand test on each and the pillow from home, gives a fair comparison. A call first tells you which ones are set up, and you can ask for me.
For a Sealy coil bed, our Campbell River store carries the Sealy Plus Jayden 14 Firm, a wrapped pocket coil Euro top. It shows no online price, so a call to (250) 286-0868 before the drive gets our best price. Guides for other sleep positions are on one page, and the medium and firm beds carry their feel on each listing.
