Best sleeping position: side, back or stomach, and what HealthLink BC and the research say

Natura Dream Mate pillow with granulated latex fill - Merit Home Furniture

Going by HealthLink BC, the best sleeping position is one that keeps your neck straight and your back in a neutral line through the night. I sell beds and pillows, not medical advice, and the published sources point most adults to the side or the back. No Canadian health body names a single winner, but HealthLink BC tells people with neck pain not to sleep on their stomach with the neck twisted.

Sleep posture is the shape your body holds while you sleep: where the head, spine, hips and legs sit against the mattress and pillow. I co-own Merit Home Furniture, which has four stores on Vancouver Island, and I work the floor at our Duncan store, selling mattresses to Island sleepers. Every health point that follows comes from a named source, HealthLink BC, the Canadian Medical Association Journal or a published study, and HealthLink BC's warning signs are near the end.

What is the best sleeping position for most adults?

Sleeping on your side or your back, with the neck kept straight, is where the Canadian advice and the research both lean. HealthLink BC's page on sleep positions for the neck asks for a pillow that keeps the neck straight in either position.

In a 2021 study in PLOS One, Australian researchers filmed 53 adults in their own bedrooms for two nights. People who woke with neck pain or stiffness spent a median of 185 minutes in what the authors call provocative postures, against 84 minutes for people without symptoms. Provocative meant lying face down, or on the side with the top thigh pushed forward so the spine twists.

The authors say a study like theirs can't show whether posture causes the pain. Going by earlier research, they add, adults without pain shift position roughly 12 to 20 times a night.

How can I improve my sleep quality?

HealthLink BC's page on sleeping better puts the sleep most adults need at 7 to 8 hours a day. Its tips start with a quiet, dark bedroom at a cool temperature for sleep. They also include the same bedtime and wake-up time every day and no caffeine for at least 6 hours before bed. If pain or anxiety often keeps you awake, it says to talk with your doctor.

Fetal, log, yearner, soldier, freefall and starfish: six named sleeping positions

The six names come from a 2003 poll of 1,000 people in Britain by Professor Chris Idzikowski of the Sleep Assessment and Advisory Service, as BBC News reported it. It was a survey, not a clinical study, and its personality readings can be ignored.

Position Shape, and share of the poll
Fetal On the side, curled with the knees drawn up; 41%
Log On the side, both arms down along the body; 15%
Yearner On the side, both arms reaching out in front; 13%
Soldier On the back, arms pinned at the sides; 8%
Freefall On the front, hands around the pillow, head turned; 7%
Starfish On the back, arms up around the pillow; 5%

The three side positions took most of the votes. The freefall is the stomach position, twisted neck and all. Idzikowski linked the soldier and starfish, both back positions, with snoring, which fits what HealthLink BC says about back sleeping and apnea below. Rolling onto the side is the first of the home fixes for ordinary snoring.

Side, back and stomach sleeping compared by what the sources say

HealthLink BC doesn't name one best sleeping position for back pain: it suggests a pillow for each position and says to find what helps you most.

Position What the named sources say
Side HealthLink BC lists it as a home step for mild sleep apnea, with a pillow between the knees for a sore back. In a 2022 reflux study, acid cleared fastest on the left side.
Back HealthLink BC suggests a pillow under the knees for a sore back, but says back sleeping may make apnea worse. CMAJ authors advise against going to sleep on the back from 28 weeks of pregnancy.
Stomach HealthLink BC's neck pain page says not to sleep on your stomach with your neck twisted or bent. The PLOS One study counted it as a provocative posture.

What's the unhealthiest sleeping position?

Of the three, stomach sleeping draws the most warnings, mainly for the neck. The mattress side of the other two is in Merit's guides to mattresses that let a side sleeper's shoulder sink and beds that hold a back sleeper's lower back. The medium-firm trials for sore backs are in what a mattress can and can't do for a bad back. A sore back in the first weeks on a new mattress may be the break-in rather than the position.

Best sleeping position for neck pain, and the pillow height that goes with it

For neck pain, HealthLink BC's answer is a back or side position that keeps the neck straight, on a pillow neither too high nor too flat. On your back, the pillow shouldn't push the head forward. On your side, the nose should line up with the centre of the body. If the neck is worse in the morning, its neck pain page says to check the pillow and the position you sleep in.

Natura Dream pillow filled with granulated Talalay latex - Merit Home Furniture

The right height changes with the position and the shoulders, and Merit's guide on picking a pillow by height and fill works through it. Two pillows listed for our Duncan store let you change the height at home, because the fill comes out.

Pillow Fill, and queen price
Natura Dream Granulated Talalay latex that can be removed to suit you; $89.00
Natura Dream Mate Granulated latex for adjustable support, in a wool-quilted cotton shell; $119.00

Both prices are Merit's, before applicable taxes, read on November 1, 2026. Neither pillow treats neck pain; they just make the height easier to get right.

Sleep apnea best sleeping position: what side sleeping can do

Sleep apnea means breathing stops for short periods during sleep, in HealthLink BC's words. Its page says sleeping on your back may make it worse, and that sleeping on your side may help mild apnea. Its other home steps include propping up the head of the bed and avoiding alcohol and sedatives before bed. It says apnea is often treated with a CPAP machine or another machine that keeps the airway from closing.

How much the side helps seems to depend on severity. A 2005 study in the journal Chest went back through sleep studies from two centres in Buffalo, New York. Apnea that came mostly on the back, and cleared up off it, showed up in 49.5% of patients with mild apnea, 19.4% with moderate apnea and 6.5% with severe apnea.

HealthLink BC says a doctor may suggest a sleep study, and sleep studies are how the Chest team sorted positional apnea from the rest. To raise the head of the bed, blocks under the frame work, and so does a base. Merit's guide to choosing a base that raises the head compares the kinds. Of the bases Merit lists, the Ashley head-only power base is on display at our Duncan store, at $764.00 for a queen. Whether raising the head suits your apnea is for your doctor to say.

Best sleeping position for acid reflux and digestion

For acid reflux, the research points to the left side. A 2022 study in the American Journal of Gastroenterology followed 57 patients sent for reflux testing in Amsterdam. Overnight, it tracked their sleep position and the acid in the esophagus, the food pipe. Acid exposure was shortest on the left side, and acid cleared in a median of 35 seconds there, against 76 seconds on the back and 90 seconds on the right.

It's one study of people already being tested, so it points a direction without settling it. HealthLink BC's GERD page suggests raising the head of the bed 15 to 20 cm, with blocks or a foam wedge, and says extra pillows won't work. It also says to wait 2 to 3 hours after eating before lying down. I couldn't find a Canadian health body that names a position for digestion in general, so the reflux work is the closest answer.

Best position for pregnancy sleeping: the side, and the 28-week mark

For pregnancy, HealthLink BC suggests sleeping on the side with pillows under the belly and between the knees, and prefers the left side later on. On the right side or the back, it says, the weight of the uterus can partly block a large blood vessel that runs in front of the spine.

A 2025 article in the Canadian Medical Association Journal puts dates on it. Before 28 weeks, its authors write, current evidence suggests sleeping position doesn't affect how a pregnancy turns out. From 28 weeks, they advise against going to sleep on the back, and say the right side appears as safe as the left. The going-to-sleep position counts: people who drift off on their back spend, on average, 50% of the night there.

The Canadian Chiropractic Association's advice for pregnant patients adds a pillow to hug, so you don't roll forward. The CMAJ article also describes a trial of 35 pregnancies in which regular pillows kept people off their backs about as well as pregnancy pillows did. Since HealthLink BC and the CMAJ authors differ on the right side, left or right is a question for your doctor or midwife.

Improving circulation at night: legs, feet and what to wear to bed

For circulation at night, HealthLink BC's advice depends on the cause. Venous insufficiency is a condition where damaged valves let blood pool in the leg veins. Its home care list includes propping the legs above the level of the heart when lying down. In Raynaud's phenomenon, cold narrows the small blood vessels in the hands and feet more than it should. There, the advice is to stay warm, wear layers and avoid tight clothing.

Pillows under the calves raise the legs, and so does a base that lifts the foot end. The Restwell Peak base, listed for Duncan and Port Alberni, moves the head and the foot and costs $999.00 to $1,099.00 before applicable taxes, by size. A base from Merit is furniture, though, and a leg that swells or aches is worth a doctor's look first.

Does sleeping with socks help?

Socks may help in a cool bedroom, going by one small study. In 2018, researchers in Seoul had six young men sleep at 23 degrees Celsius, once with bed socks and once without. With socks, they fell asleep 7.5 minutes sooner on average and slept 32 minutes longer. Six people is a tiny sample, so read it as a hint. Skip any pair tight enough to dig in, since HealthLink BC's Raynaud's page warns that tight clothing can cut blood flow.

From the showroom floor

If a doctor has asked you to sleep on your side or with your head raised, try beds and pillows that way. Stay longer than feels polite. I can't tell you which position suits your body, but I can help you find a bed and pillow that suit the one you've been given.

When a sleeping position problem needs a doctor, not a new pillow

No pillow, base or mattress at Merit treats a medical condition. HealthLink BC sorts the warning signs below by how fast to act, and its advice comes before anything in this guide.

HealthLink BC says When
Call 9-1-1 Neck pain comes with chest pain or other heart attack signs, such as shortness of breath and nausea.
Call 9-1-1 Sudden shortness of breath, sharp chest pain that can worsen with a deep breath, or coughing up blood, the signs it lists for a clot in the lung.
Call your doctor now Pain wakes you from sleep, or numbness or weakness in the arms or legs is new or getting worse.
Call your doctor now Swelling, warmth or tenderness in a leg, or leg pain that gets worse when you stand or walk.
Call your doctor You or your partner snores loudly and heavily and feels sleepy in the day, or stops breathing, gasps or chokes during sleep.

For anything in between, 8-1-1 reaches HealthLink BC from anywhere in the province, and its nurses answer 24 hours a day. People who are deaf or hard of hearing can call 7-1-1.
